Your First Dental Visit

If you’re new to our office, thank you for trusting your oral health and hygiene to our caring and knowledgeable dentist and team. At your first Smiles of Springfield appointment, you’ll be greeted warmly upon arrival. We’ll answer any questions you may have, take you on a tour of the office, and introduce you to available team members. Your treatment will consist of a very thorough diagnostic screening and professional teeth cleaning. Then, the doctor will sit down with you to discuss your plan for ongoing care. The goal of this first visit is to establish a complete picture of your current state of oral health and to partner with you to develop a treatment plan that meets your needs, schedule, and budget. Patient Forms

Before your appointment, we encourage you to complete all the new patient information forms from the comfort of home. It’s never fun juggling clipboards and insurance cards in a waiting room, and we want to maximize your convenience in every aspect of dental care. You can download your forms here. If you have any questions as you complete the forms, please don’t hesitate to reach out to our knowledgeable team members. We’ll be happy to help anyway we can. You can also complete your paperwork in our office, but please allow about fifteen extra minutes.

Dental Insurance Accepted

We know our patients invest in their dental insurance coverage, and we want to help them maximize that coverage. We are in-network providers with numerous local insurance providers, which means patients already receive their maximum benefit from treatment in our practice. We are also happy to process and file claims for out of network benefit plans. Don’t hesitate to ask any questions you may have about your coverage.  We are in-network dental care providers for all of the following insurance plans:

Payment is due at the time of service for the out of pocket costs associated with your treatment, and we accept payment in the form of cash, check, or major credit card. For those who wish to spread the cost of care out over several months, we’re happy to work with CareCredit to provide low and no interest payment plans. You can complete the online pre-approval form at home, or we’ll be happy to talk to you about CareCredit at your next appointment.
